Financial & Booking Policy
Before booking your appointment, please read and fully understand the terms and conditions outlined below. This will serve as the financial and booking agreement between you (the patient/client) and Perrigo Massage LLC, doing business as Body Works Massage Clinic, prior to receiving services.
Please be aware that health insurance coverage is not a guarantee of payment. When booking online, ensure that you have received a confirmation of your appointment.

Insurance Denials:
I understand and agree that if my insurance company denies coverage for any reason, I will be financially responsible for the full payment of the services rendered. -
Co-pays and Payments:
I understand and agree to pay any applicable co-pays or financial portions at the time of my visit. -
Late Fees:
I understand and agree that if my balance becomes past due, I may be charged interest or late fees until the balance is paid in full.

Co-pay or Deductible
​
You will be responsible for paying your co-pay or deductible at the time of your visit. To verify your co-pay, coinsurance, or deductible amounts, please contact your insurance provider prior to your appointment.
Non-Reimbursements
​
If your insurance does not cover the service for any reason, you will be responsible for the amount due based on your insurance plan’s coverage and premium.

I understand and agree to pay any amounts due for services rendered, and I also agree to cover any charges in the event that my insurance denies the claim. I acknowledge that I am receiving the service and take full responsibility for any costs not covered by my insurance.

Cancellation or No-Show Policy
​
To avoid a cancellation fee of $60.00, please reschedule or cancel your appointment at least 24 hours in advance.
Your health and well-being are our top priority, and we aim to provide you with quality care in a timely and focused manner. We understand that life can be unpredictable, so if you need to reschedule or cancel, we ask that you notify us by phone, text, email, or online at https://www.bodyworksmc.com at least 24 hours before your scheduled appointment.
If you cancel or reschedule 24 hours in advance, no late or cancellation fees will apply. However, if you fail to show up or cancel within 24 hours, a $60 fee will be charged.

Why?
​
When you book an appointment, we reserve a treatment room and block time specifically for you. The $60 fee covers the time of your massage therapist and the lost opportunity for the business. By providing at least 24 hours' notice, you help us offer your reserved room to another patient in need of the service.

We value your time, and we appreciate the same courtesy in return. When an appointment is made, we prepare your treatment room and review your records to ensure a high-quality visit.
Booking Online
​
When booking online, BWMC requires a minimum of 2 hours' notice. If you see an available spot and wish to book for the same day, we recommend calling or texting to confirm availability.
​
Please note that booking online does not guarantee an appointment until BWMC personally accepts or declines the request. If you do not receive confirmation and show up for your appointment, we apologize for any inconvenience. Due to preparation time, there is a very slim chance of being accommodated within a 1-2 hour window.

For the most reliable booking experience, we recommend scheduling at least one week in advance.

Financial Agreement
​
I understand and agree that the terms and conditions outlined in the financial agreement between me and Perrigo Massage LLC, d.b.a Body Works Massage Clinic, obligate me to pay for any services rendered in the event my insurance company denies or does not cover my visits.
​
Furthermore, I acknowledge that my health insurance is not a guarantee of payment, and I am fully responsible for any outstanding balances.
