Before booking your appointment, please read and fully understand the following terms and conditions. This document serves as the financial and booking agreement between you (the patient/client) and Perrigo Massage LLC, doing business as Body Works Massage Clinic, prior to receiving services.

Health Insurance Disclaimer

Please note: Health insurance is not a guarantee of payment.
If booking online, ensure you receive a confirmation before arriving for your appointment.

Policy Agreement Insurance Denials

I understand and agree that if my insurance company denies coverage for any reason, I am financially responsible for full payment of all services rendered.

Co-Pays and Payment at Time of Service

I agree to pay all applicable co-pays, co-insurance, or deductible amounts at the time of my visit.

​

Late Fees

If my balance becomes past due, I may be subject to interest or late fees until the full balance is paid.

​

Co-Pay & Deductible Responsibility

It is my responsibility to confirm my co-pay, coinsurance, or deductible amounts with my insurance provider prior to my appointment. These amounts must be paid on the day of service.

Non-Reimbursed Services

If my insurance does not reimburse services for any reason, I agree to pay the full balance due, based on my insurance plan's coverage and limitations.

I understand and accept full financial responsibility for any portion of services not covered by my insurance plan. I acknowledge that by receiving care, I am agreeing to pay for it.

Cancellation & No-Show Policy

To avoid a cancellation fee of $60.00, please cancel or reschedule your appointment at least 24 hours in advance.

We value your time and aim to provide every client with focused, quality care. If you must cancel or reschedule, kindly notify us by phone, text, email, or online at www.bodyworksmc.com at least 24 hours before your scheduled session.

• Cancellations made 24+ hours in advance: No charge.

• Late cancellations or no-shows: A $60 fee will apply.

Why This Matters

When you book an appointment, a treatment room and therapist’s time are reserved specifically for you. Last-minute cancellations result in lost opportunity for both the therapist and other patients. Your timely notice helps us serve others in need.

Online Booking Policy

​

Online bookings require a minimum of 2 hours’ notice for self-pay appointments and at least 2 days’ notice for insurance appointments. Please understand that insurance bookings require additional time to verify your insurance information and eligibility.

​

For insurance appointments, please provide a copy of your insurance card when submitting your booking request. You may text a photo of your insurance card to 253-630-6614 or send it through the website’s auto chat box, located in the lower-right corner of the screen.

​

Important: Online appointment requests are not guaranteed until you receive a personal confirmation from BWMC. If you do not receive a confirmation, that means your appointment has not been secured. 

​

For the best availability, booking at least one week in advance is recommended.
